3. the 5.1V reference on the PCB is undoubtedly for a protection zener placed right next to the connector. The input undoubtedly uses three pins in order to support balanced and single ended inputs ie likely to be +in, -in, and ground.connector maay be a JST connector similar to the tiny fan connectors some products use but I can't say for sure.
4. probably DC offset null.
5. nothing wrong with stepping back to a, safer higher impedance load for testing the thing. once you are successful getting it powered up and driving a tiny load then try more realistic larger loads.
