Actually, it's A below middle C that's 220 Hz, according to the well-tempered scale. And in fact, pianos aren't really tuned to the well-tempered scale, but this is another issue ;-)

Yes, the odd-order distorsion is very painful. If one relates it to music theory and harmony, it's very obvious. 2nd harmonic is only one octave above the fundamental, and is very closely related. But harmonics like a 3rd or 7th sound very nasty. Like someone in a choir who would insist on singing a parallell 3rd or 7th along with the melody.....ouch!! Although, one has to have a very good ear to be able to do that ;-)
