A lot of issues here. First off, it's never a good idea to Y two outputs together. You need to put some resistance in series with each before joining them together. 10k ohms should be enough to prevent problems. If you go with the Harrison filters suggested by one of the posters, use 2 and Y their outputs together. That will also serve to isolate the two outputs from your amp.
Another good possibility is to tap off the L & R speaker outputs of your main amp, through simple resistive voltage dividers, and then from the juntion of the outputs of the dividers, go through a 2nd order RC filter to feed the amp that drives the Wharfedale. I have used this approach several times with very good results and it's inexpensive to implement. A level control can be incorporated into this option.
Now a question about the woofer amp. Does it have input level controls? Because you'll need some way to match the level of the sub to your main speakers. If it's an integrated amp with a preamp section, you can use the bass control for some bass boost if needed.
