Thorsten, I was going to recommend that article as well. Barrie takes a cheap shot at Dr. Otala, but oh well! I met Barrie Gilbert at a conference in 1974 and tried to explain to him some of Matti Otala's ideas. He thought I was crazy. Now, of course, he knew it all the time ;-)
Still, this is a great article, BUT the diagrams are not reprinted. This is going to make it REALLY difficult to understand.
What is really outstanding in this article, is section (14) where phase modulation (jitter?) is created by internal amplitude modulation. This is really scary! This is also why I built my last preamp without any global feedback. By the way, Matti Otala wrote a paper on this phase modulation more than 20 years ago but the AES refused to publish it (I'm pretty sure) Still, it was given as an AES lecture and there is an AES preprint. This is much like Jon Risch, who has an excellent AES preprint, but it wasn't included in the Journal.
