I suspect two issues. Transformers could have been designed for 115 volt primary causing somewhat higher plate voltage on 120 volts AC primary & Eico may have 'pushed' the EL84 tube voltage ratings. With cathode bias, B+ to ground is 12-14 volts higher than the plate to cathode voltage within the tube.
