RE: noise

When you say "Sounds like 60hz", and you've tried "grounding schemes" etc. - You should first identify whether you are dealing with Ground Loops or not.

The easiest way to do this, is lift the grounds on the amp and pre-amp with a cheater plug. If this removes most or all of the hum, you at least know you have ground loop problems to solve. (Which could be most all of your problems).

If this doesn't help another area that I have found that causes hum are cold solder joints. If this is a recent build and you have not done this already, you can re-visit and re-flow your solder joints.
