Assuming the tubes are all good, my guess is that your cathode resistor is problematic.
It could also be in the cathode bypass cap - try disconnecting the cap.I would recommend installing a screen stopper on the output stage that is (minimally) of slightly higher value than the primary dcr of the OPT.
