I may have missed something in your post, so hopefully it's not presumptuous for me to ask: Have you verified that the 12AX7 filament is correctly connected? If as you say you're using 6.3VAC, it should be applied to both filament sections in parallel for each 'AX7, i.e. pins 4 & 5 jumpered and 6.3V between 4/5 and pin 9.
