This one uses EL95s since I only wanted 5 watts. The OPTs are ultralinear. I used a CCS in the cathode circuits of the 12AT7s. I added feedback, but only to get rid of too much gain. It still has enough gain that my FM tuner has enough output to clip it if I run the volume up too high. So it has 2 inputs and a volume control.
