All that is a tall order. I was looking for something similar and couldn't find anything. Cary makes a 300B amp with everything on your list but the internal phono, the CAD-300SEI. I though about it, but already have a 300B integrated (no remote, which I really needed) so I went with a different option, a Linear Tube Audio ZOTL Ultralinear +**. Not class A, 20 watts, remote, outstanding headphone section (2 actually) and it gives me almost everything my 300B does sound wise in a nice, and since there are no traditional output transformers, very light and manageable package, pricey though. I got a demo model and saved a bit. Internal phono stage available, but I didn't get one. Oh, and it's made in Maryland, USA! I'm very satisfied with it so far powering my Avalon Acoustic Symbol speakers.Good luck in your quest, but there's just not that much out there with ALL your requirements. The 211, 805 and 845 integrateds are few and far between, very expesnive, very heavy and run hotter than Hades, none of which I was interested in dealing with.
**note, it is NOT an OTL. It's a David Berning design that doesn't use traditional output transformers.
