I read an story about a year ago written by one of the lawyers involved in the SRV incident litigations.

Based on re-creations of the accident, it is believed that the ground fog was not the cause, and at best a minor contributing factor. What they discovered was that unknowingly, Stevie managed to jump into the helicopter piloted by the least experienced pilot they had, and that this pilot had no instrument certifications and should not have been flying that night.

Causation was attributed to lifting off from a very brightly lit parking lot, and once above the lights and the ground fog they were in total, complete darkness. The pilot then transitioned to horizontal flight before his eyes adjusted to the darkness without realizing he hadn't ascended high enough to clear surrounding hills. He flew at near full speed into a hillside less than a mile from where they took off.

I'll see if I can find that story again. Somehow reading it made the whole thing seem even more tragic.
