What kind of monitor are you using? I found some complaints that appear to be issues with TVs as monitors, possibly with Samsung or using 4K TVs. Other strange things were mentioned about possibly 11.3 or having the monitor not respond after an update.
Apple claims to do this:
If your display isn't recognized
If your Mac doesn't recognize your HDTV, display, or other HDMI device after making the connection:
Turn off the HDMI device while your Mac is turned on.
Unplug the HDMI cable from your Mac, then plug it in again.
Turn on the HDMI device.
But it seems like you tried that.
If nothing else works, I'd try a thunderbolt to hdmi adapter instead of the usb.
