Could be all of the above.

Most virologist believe it to have evolved in animals. No evidence in its genetic makeup to suggest otherwise.

Could have escaped from a lab that was studying it, or the animal it had infected, by infecting a lab worker who spread it to others.

The conjecture that it was artificially created as a possible biological weapon, as stated by many, is still not at all likely.

That it was a naturally occurring virus in animals that that somehow jumped to humans is still the most likely scenario.

Whether it happened inside or outside a lab is not yet known.
