Trouble is? Nothing is '100% safe'.

I suspect the calculation is mesured in 'incidents per year, per mile' or some other metric.

Don't forget, even an otherwise perfect pipe, you still have valves with packings and seals.

MTBF for a pipline as is described in the original post, should be in the hundred years + range.
