I swear that many opioids and psychotropic drugs literally rewire the brain to such an extent that many, many of those addicted don't even know it, all they know is their pain, what [can] relieve it, and where to get it ... there are those that suspect they have a problem but self denial is pretty potent too [aside from hard core abusers set to self destruct]

but the prescriber knows, or should ... yet the addict used to be able to 'doctor shop' before being 'forced' into street drugs

the laws finally caught up to these practices and realities, and yes, 'Big Pharma' was in the background incentivizing those sales even to doctors with the best of intentions, so they'd look for another with $$ in their eyes

you, I, and others may have enough intellectual and self identity wherewithal to manage our health care amongst other aspects of life, but a surprising majority don't and in addition see doctors as authority figures not to be questioned

this spiel is not intended to take anything away from the points you've made, just kind of round out the big picture a bit ...
