I've been very happy with Ooma. One time fee for the box is ~$100, and then like $6 a month on credit card, which is mostly regulatory fees. I have no extras, just basic service which gets us unlimited domestic long distance as well as local calling and voicemail. Voice quality is very good. In most cases as good as my POTS line which I also maintain for old home security system (and solicitors it seems). VOIP is not perfect yet, but getting very close, IME.
