The 85% figure is for pre-exposure prophylaxis . Since smallpox was eradicated many years ago, it's not actually known that persons vaccinated 50 years ago are protected. Current recommendations are that folks who received smallpox (vaccinia) vaccine greater than 3 years ago get re-vaccinated if they are at risk.
Bear in mind that some of the at risk persons may be immunosuppressed because of HIV.
