You don't NEED the battery box as it makes the whole setup more bulky but it does provide a nice selection of outputs and protection from the weather if used outdoors. You can also use a smaller capacity battery for a CPAP [maybe 20-Ah to 30-Ah] depending on whether you use the humidifier or not.My wife's CPAP operates from 24vdc so I found a 12v-to-24v DC-to-DC converter with cigarette lighter style plug. There's a Resmed branded unit for use with her Resmed CPAP but I found an aftermarket version for 1/3 the price. We'll test it out this week.
DC to DC should be a little more efficient (less battery capacity wasted) vs DC to AC to DC using an inverter. But with a slightly larger battery it really should matter that much.
