Another possible option is presented by Morgan Jones in the 4th edition of his book Valve Amplifiers page 129. He tongue in cheek refers to it as a "beta-follower" as apposed to a" mu-follower". By substituting a BJT for the large value load resistor in the typical mu-follower circuit it lowers the required budget for B+ voltage for the stage and increases the impedance seen by the lower tube. Also, the grid of the upper tube is now direct coupled to the output of the lower tube, no capacitor. In the mu-follower and the Gary Pimm example I believe there is a capacitor connection required.
Hint to Kevin: Please make me a board.
