Hi,
I order to get that kind of efficiency, I would use 2 efficient woofers connected in parallel. This will not only give you higher efficiency but also make the front horn shorter for a given cut-off. I would not fold the horns when using the horns up fairly high.
I have large, straigt 29 Hz bass horns each loaded with double 15" PD.158 Precision Devices woofers. According to simulations these are close to 110 dB in sensitivity.
