I'm not sure if you have seen Tom's more recent designs like the TD-1 or his new SH-50 with Danley Sound Labs, but the problems you described are a concern, and dealt with. The simple answer is that below some low frequency, the throat is close enough to not allow a reflection, but rather just additional air volume in the horn. Indeed, at roughly 1/4 W-L to the throat, there will be a notch in the response. The small entry holes and remaining air space in front of the cone makes for a bandpass that can be made to greatly attenuates output where reflections would occur.
This does generally place a bit more demand on the LF output of each section, but the ability to employ larger horn (relative to the higher frequency components) in a given box size helps counteract this a bit.
The design is hardly without intricacies and interactions to be dealt with, but having measured many of them, I suspect the problems you are much less significant than you may suspect. The intrusion of the midrange entry holes are probably the only identifiable issue, depending on how the design was optimized.
