Perhaps you didn't read the article I linked to. It explicitly covered the
issue: "It is the editor's decision whether a product distributed by mail-
order or via the Web qualifies for review. However, at minimum the
company must have a formal US presence and must offer a 30-day,
money-back refund policy."
Seems clear enough to me.

Yes, when those products are sold through the traditional retail chain
rather than direct. That number since 1989 has been "5," as you would
have learned had you actually read the linked article. I chose 5 as it's
a trivially easy number of dealers for an established brand to have but
a difficult target for a start-up.
