I find the Etymotic musician earplugs to be life-safers, and they don't seem to kill the highs too much. They are also so cheap anyone can get a couple pairs so they always have a pair on them - as I am very forgetful that's what I do. One pair stays on my keychain.

Yes, I've also found joints where they are not enough, and that's an immediate sign to get out, go some place civilized, have a martini and a good dinner. And yes, it's a shame when great musicians' performances are killed by too much volume, and I hate to waste a the price of a ticket, but if it's so loud it hurts, it's time to cut your losses.
