Suggest a third party spam filter, like Spamfighter. Search for thunderbird antispam and you'll get lots of options. The important qualities for me are the ability to create a white list from my address book, and maintain my own black list. (Some apps have only communal blacklists.)

A different approach is to create your own domain name and put it on a hosting service. I use Verve Hosting, which charges $5/month for a domain (btw outstanding good service and customer support.) With the domain, I can create 5 (I think) email addresses. I can use POP or IMAP protocols for incoming email, and SMTP for outgoing, and they support direct web access, so you can use any email app or just a browser to get your email. The big help comes with BoxTrapper, a common hosting spam blocker. If you turn it on, it lets you maintain your white list, black list, and lets you easily review your daily email queue. Emails from unknown parties get a challenge (like "enter the numbers in the box to prove you are not a bot") and if they do not respond they email is blocked. About once a week I log in to Boxtrapper and take a quick look at the queue for each day to see if I want to whitelist something good that got blocked. (When I email someone their address is automatically white-listed.) Screenshot of daily queue:

It takes a couple minutes to go through the last week or so. I'll get maybe one spam email every few months, when there is a spammer who goes through the trouble to respond to the challenge. And there's a bonus: because it supports IMAP mail, I can do email on my computer or mobile device, and it keeps mail sync'd between all devices while letting me handle email offline if I want. (I know you can do that with gmail et al., but their offline interfaces all seem really kludgy to me.)
