The rating of A+ was originally introduced for digital processors that were regarded of top quality and that could handle DSD or 24/96 LPCM data.
After awhile, some turntables started to appear as A+ without any description of why a grade of A was deemed insufficient. And, if there are A+ turntables, then why not add grade inflation to other categories as well.
