When I ship to Canada we put the actual shipping charges on our account (yes, not significantly more than shipping to the US) and all clearance charges are payed separately by the recipient. If they have an account with UPS, they get billed, if not, like if they are a consumer, those clearance charges must be paid upon receipt.
I could see where there is a possibility that's what happened in this case.
