The outlet in the photo is a Hubbell HBL 5262W, that is the 15A version with straight wipers. This is actually the best choice for most people because there are very few 20A wall plugs in the average .......

I think the Hubbell HBL 4PLEX 20 along with the Hubbell HBL 4API adapter plate is pretty much what the OP is after, it certainly is the best solution. This will give you 4 hospital grade outlets right .......

I snip the end of the shield clean with the side cutters when cutting it to length and try to keep it from fraying during installation. The clean, unfrayed end makes a tidier looking finish. Gene ::: .......

Choy, You can use the the PTFE 14 tubing to space the shield, it is nearly the same OD as the 89259/1506a cores. Just combine two pieces of the PTFE 14 with the two cores in a 12:00/6:00, 3:00/9:00 ar .......
