[ If we assume the picture represents the load end of a cable, and the telescoping shield (1) is not attached at the other end of the cable (floating), do you attach a 0.01 uF capacitor between the telescopic shield and the RCA barrel at the load end? Is this correct? ]

Not quite. You would place the 0.01 uf DISC CERAMIC cap as you stated, but the source end of the extra shield would be connected to ground.

What this type of connection does, is provide a "floating shield" within the operating band of the cable, yet terminates the shield at higher RF frequencies.
