Possible causes:
1. Arm board or pillar not mounted perpendicular
2. Bearing yoke not perpendicular
3. Arm or cartridge platform twisted
#3 is not unknown with Rega arms, I had a Moth (OEM RB250) with a twisted cartridge platform. IIRC it has been said that Rega weren't so QC conscious when supplying arms to others and that they kept the better tubes for RB300 and upwards.
When Michell prepare their RB250 armtubes for converting into Tecnoarms, they machine the underside so that it is perpendicular. I checked mine before buying it.
