Like many of us, I had assumed that the relevant diameter would be at inner surface of the belt but now that you have pointed it out, I see that its actually part way through the thickness of the belt.

It took me a while to wade through the whole thread so for the benefit of anyone else reading this, I've included a link to the post that seemsed to sum it up simply and clearly.

For anyone who has trouble imagining this, it might be helpful to think about how a rubber belt with an inelastic backing would behave. If the backing is on the outside and doesn't stretch then the rubber will compress as the belt goes around the pulley and the effective diameter will be at the outside of the belt. If the backing were on the inside the effective diameter would be at the inside of the belt. A rubber belt with no backing will behave somewhere in between. To find out exactly where, they can either believe you or read the paper that you referenced.
