No - not for me... Here's my setup...

Here's my HTPC setup:

- Core 2 Quad Q6600 CPU (previous main PC) - 2.4GHz, undervolted to reduce heat
- Noctua CPU cooler
- 8G DDR2 RAM
- nVidia GTX 560 graphics card - hacked BIOS to allow slower fan speed down to 25% with low activity/cool temp
- 4+TB HD storage for movies and music
- Blu-Ray / HD-DVD drive
- HDMI connection to 3D TV - HDMI 1.4 for 3D Blu-Ray
- Gigabit wireless router
- Windows 7 64-bit

On this computer I also run Apache web server for work, an FTP server, and Squeeze Center for my Squeezebox units (SB3, Boom, Transporter, my dad's Touch across town).

I have it all in a Fractal Design case to keep the noise down. It's very quiet and you can slightly hear the graphics card spin up when playing 3D games. When idle or just serving up music, essentially silent. Since my music is streamed through the Squeezeboxen thru the house, I don't feel there's any issue with reduction in music quality.
