Wrong. My comment was specific to the 5 volt issue. Abe's comment below sums up the situation. If the DAC can't filter the 5 volts it shouldn't use it in the first place.
USB is somewhat baroque in design and has its issues, but it works reliably when the equipment and cables meet the USB specification. However, there being a long tradition of manufacturers of high end audio components disdaining sound engineering practices (whether due to lack of knowledge or resources to procure required test equipment and other tooling), it is not surprising that some USB audio products don't work reliably.
