On the Apple II, the audio hardware was a single program addressable flip flop that could be toggled by software. The flip flop was directly connected to a cheap speaker. Audio could be generated as pulse density modulation. (Pulse density modulation was an essential part of generating two tones simultaneously on the Apple II and this was a part of the Phone Phreak culture of Woz and Jobs.
I programmed my Apple II to synthesize precisely controlled pitch (down to .0001 Hz). This was done by counting out the instruction times and the code generated a square wave sampled at the machine clock cycle, 1 Mhz. (This was about 5 times faster than the instruction rate of the 6502 processor.) To get precise pitch with no birdies, the samples were dithered so that a timing between transitions could be a fraction of a clock sample. I was interested in exploring different musical scales, the various ways of tempering the notes.
