I have owned both, the Ultimate has lower hum when connected to high-impedance inputs and more bass than the original First.
VDH increased the number of carbon fiber strands both on the shield and inner conductor, the Ultimate has much lower series resistance than the First.
It sounds slightly more transparent, dynamic & quiet than the First.
Both are outstanding as ICs between a digital source and the amplifier/preamplifier, the conteract what some describe as digital's "metallic" sound, specially on older generation CD players.
