The source selector switch may be the cause of your problems. The switch they used initially in the A28B and A48B and I believe the A21A, is known for giving problems. They later changed to a different switch.I would open up the amp, give the selector switch, tape switch and mono switch a good blast with Deoxit, then operate the switches about 20 times, close the amp and fire it up. There is a fair chance that the problem will have gone away.
If that cures it, the long term solution is to order the new switch type and replace the old switch.
