The closest things I've seen is the Furutech ADL GT40 which has what you want but no power amp (It's a preamp/DAC with MC and MM phono stage and headphone amplifier) http://www.whathifi.com/review/furutech-adl-gt40
Grant Fidelity has "Exactly" what you are after. Full integrated amplifier and DAC (The DAC board can be replaced when newer higher sampling models become available) MM and MC phono stage, tube headphone amplifier, and it has preouts. And it's under $2k.
Downside is it uses tubes (well that depends on you). On the upside if you're a SS guy at least it uses KT 88s in class AB 1 ultralinear which is as SS sounding as tube amplifiers likely get). If you're a tube guy - the downside is that it's as solid state sounding as amplifiers likely get.
Still, it's hard not to be impressed by everything it can do - and it did win product of the year in enjoythemusic.com and has gotten good press elsewhere.
The Chinese stuff is absurdly well built too so there is zero fear on that front - many offer longer warranties as well.
I'd also explore what other ones are doing - remember just because their name brand recognition in the U.S. may be limited doesn't mean much since many of them OEM for big name brands and have done for 20+ years. Have a look at the ridonculously built Consonance CD players, Line Magnetic, Audio Space, Melody etc.
