Hmmm.... Just got back from vacationing and traced the signal paths on the evaluation board. Indeed, the center tap needs to go to the "DG" terminal (I assumed DG was digital Ground and the 5V was supplying the digital DSP chip). I can find no direct reference to placing the -5V leg to the AG but did it anyway. The hiss went away and all is well - I wonder if the other people who reported hiss in this amp did the same mistake? Anyway, thank-you for your input.
